Inspector Notes

An unannounced monitoring visit and facility tour was conducted on 8/2/2022. At the time of inspectors' arrival there were 12 preschool aged children in care with 3 staff members and 1 administrator. Children were observed interacting with staff, free play, lunch and hand washing. A sample of 5 children's records and 4 staff records were reviewed. Injury reports and medications were also reviewed. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Based on observation it was determined the center did not ensure all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition.
Evidence:
There was peeling paint/drywall on the restroom wall, exposed foam/filling due to tears in the child sized red chairs and the black stool in the 3-4 year old classroom.
Plan of Correction: A maintenance request will be submitted.

Based on record review it was determined the center did not ensure the written record of children's serious and minor injuries contained all of the required information.
Evidence:
The injury record for child #6 dated 7/21/22 did not contain the staff present, treatment given, and the two required signatures.
Plan of Correction: Staff will be reminded to document all of the required information on injury reports.
