Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews.
A monitoring inspection was initiated on 1/26/2022 and concluded on 1/27/2022. The director was contacted by telephone and a virtual inspection was conducted. There were 70 children present with 13 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations
2Evidence: Staff #4 (Date of Hire: 12/17/21) and Staff #5 (Date of Hire: 12/17/21) have both resided in Kentucky within the last 5 years. The center does not have a copy of the Kentucky criminal history background check results for either staff.
Evidence: Child #8 has a diagnosed food allergy and has emergency medication on site. Child #8 does not have an allergy care plan signed by a physician on file.
