Inspector Notes
An unannounced complaint inspection was initiated on 09/22/2023 from 9:55am to 12:05pm in response to a complaint regarding neglect and injury reporting. Interviews were conducted with the staff, documentation was reviewed, videos were observed and the playground was observed. Based on the preponderance of evidence collected during the investigation, the complaint was determined to be not valid. There were 2 violations discovered during the investigation that were unrelated to the complaint. The exit interview was conducted on 9/29/23 with the Director. Areas of non-compliance are identified in the Violation Notice.

Violations
2Evidence: On the Kindergarten playground, at the top of the slide where it attaches to the playground equipment, there is a gap of approximately .25 inches that could be a risk of entanglement of a child?s clothing.
Evidence: On the Kindergarten playground, there were 2 bolts under the play equipment that were protruding by 5 threads which presents an impalement or entanglement hazard.
