Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 9/3/24 and completed on 9/3/24. The on-site inspection began at 10:00am and ended at 12:00pm. The inspector reviewed compliance in the areas listed above. There were 61 children present and 15 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, observations, and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/10/24. A POC submitted after this date will not appear on the public website.

Based on observation, the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe, and operable condition.

Evidence:
1)The lower part of the wall in the Cardinals classroom, has large areas of paint peeling from the wall.
2) On the Twos/Threes playground, there is a sharp rock 5? x 1? and a sharp rock 8? x 3? protruding from the mulch area on the hill in the back which could pose a tripping hazard or snag to the skin.
3)On the Twos/Threes playground, there is a root 19? long x 1? thick protruding from the mulch area on the hill in the back which could pose a tripping hazard.
Plan of Correction: A handyman has been contacted to paint and touch up cardinals wall.

Preschool 2's3's playground will be evaluated by a landscaping company to remove rocks and roots.
