Inspector Notes

A monitoring inspection was conducted on 4/26/2022. There were 13 preschool children present with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program.

Based upon review of medication stored for potential administration to children and staff interview, the facility's procedures for administering medication do not include the duration of the parent's authorization for the medication to be administered.
Evidence:
1. There was Benadryl and and an epi-pen stored for emergency use for child 3. The parent authorization forms did not include the duration time frames for which the parental authorizations were being given.
2. There was an epi-pen stored for emergency use for child 4. The parent authorization form did not include the duration time frame for which the parental authorization was being given.
3. Staff 3 acknowledged that the parent authorization forms for the above did not include time frames (duration) for which the parent authorizations were being given.
Plan of Correction: The facility responded with the following:
Parents will be required to complete a separate authorization form that clearly includes the time frames for which authorizations are being given.
