Inspector Notes

A monitoring/renewal/complaint inspection was conducted on05/18/2023 . There were twenty-two children present with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations were reviewed with the school administrator during the exit interview.

Standard 8VAC20-780-240-E
Based upon review of staff records and staff interview, the facility has not ensured that staff have completed orientation training in first aid and cardiopulmonary resuscitation (CPR) within 30 days of the first day of employment.
Evidence:
1. The record provided for staff 4, hired on 1/30/2023, did not include documentation that staff 4 completed training in first aid and CPR.
2. Staff 1 confirmed that staff 4 had not obtained first aid/CPR orientation training.

Based upon review of staff records and staff interview, the facility has not ensured that documentation of orientation training includes all required documentation.

2. Staff 1 acknowledged that the documentation of the orientation training for staff 3 and staff 4 did not include the above listed required documentation.
