Inspector Notes
A monitoring/renewal/complaint inspection was conducted on05/18/2023 . There were twenty-two children present with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations were reviewed with the school administrator during the exit interview.
Violations
5Evidence:
1. The record provided for staff 4, hired on 1/30/2023, did not include documentation that staff 4 completed training in first aid and CPR.
2. Staff 1 confirmed that staff 4 had not obtained first aid/CPR orientation training.
Evidence:
1. The documentation of orientation training for staff 3 and staff 4 did not include the training topics, the training delivery method and the entity or individual providing the training.
2. Staff 1 acknowledged that the documentation of the orientation training for staff 3 and staff 4 did not include the above listed required documentation.
Evidence:
There was an epi-pen stored for emergency administration to child 1. The epi-pen expired in January 2023.
Evidence:
An injury record dated 3/21/2023 for child 2 did not include documentation of the date the parent was notified of the child's injury. In the space where a date was to be entered, the form said "yes".
Evidence:
The child in the three year old preschool classroom were served whole hot dogs in buns for lunch.
