Inspector Notes
A monitoring inspection was initiated and concluded on 05/07/2024. The inspector arrived at 9:20 AM and departed at 10:52 AM. There were 26 preschool children present with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed. Morning activities and snack time were observed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the program director, assistant and licensing specialist during the exit interview.
Violations

Evidence:
1. There are numerous exposed tree roots on the large playground, several of which a child's foot could become entangled between the ground and the exposed root creating a tripping hazard.
2. There are six areas along the back fence of the large playground where poison ivy has grown through the fence.
Evidence:
The engineered wood mulch used as resilient surfacing under the swings on both playgrounds has compressed and decomposed into a one inch depth. At least six inches of engineered wood mulch is required under and around the fall zones of the swings
Evidence:
1. There was epinephrine in a bag labeled with child 4's name. There was no prescription label or box with the required information.
2. There were 4 diphenhydramine tablets in a bag labeled with child 4's name. The tablets were not in a bottle or box labeled with dosage or time/times to be administered.
