During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 03/23/2026, as a part of the licensure period. The on-site inspection began at 8:25 AM and ended at 9:30 AM. The inspector reviewed compliance in the areas listed above. CDC: There were 30 children present and 4 staff. The inspector reviewed 3 children?s records and 3 staff records on-site.
This inspection included:

? tour of the facility,
? interviews, and
? observations.
Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on March 30, 2026. A POC submitted after this date will not appear on the public website.
Violations
2The medication authorization shall be available to staff during the entire time it is effective.
There was an epi-pen and diphenhydramine stored for emergency administration to child 5 and an asthma inhaler for use by child 6. There were no written parent authorizations for the potential administration of these medications.
All parents were notified that written parent permission to administer medication is required. Parental permissions have been obtained.
An injury report for child 7, dated 12/09/2025, did not include the time the parent was notified of the child's injury.
An injury report for child 8, dated 3/17/2026, did not include two signatures.
The procedure regarding completion of injury reports was reviewed with staff.
