Inspector Notes
A renewal inspection was initiated on February 27, 2023, at approximately 10:00 AM to 12:30 PM and on April 6, 2023 from 9:45 AM to 11:55 AM. There were 14 children present, ranging in age from three years to five years, with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care, and emergencies and nutrition. A total of five child records, four staff records and five board member records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

Violations
5Evidence:
1. The records for staff #1 (employed:10/11/18 ), staff #2 (employed: 5/28/21), staff #3 (employed: 8/05/20), and staff #4 (employed: 7/12/22) did not have documentation for the correct fingerprint based criminal history record check.
2. A member of management stated that the fingerprint documentation was not the fingerprint based criminal history record check.
Evidence:
The record for board member #2 (employed:10/18/18) did not have documentation of the correct fingerprint based criminal history record check.
Evidence:
1. In room #1, one epinephrine injection (EpiPen) was hanging in an unlocked bag on the wall near the exit door within reach of children.
2. Staff #1 acknowledged that the EpiPen was not in a locked container.
Evidence:
In room #1, child #6 was observed in attendance. Child #6 did not have a record on file at the center.
Evidence:
The records for child #1 (enrolled: 01/03/23), did not contain documentation of viewing proof of the child's identity and age.
