Inspector Notes
An unannounced monitoring inspection was conducted on 11/29/2022. Upon arrival at 10:43 a.m., there were a total of 46 children, ages five months to five years, under the care of 12 staff. The Licensing Inspector reviewed four children?s records and five staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.
The children were observed during arrival, free choice activities, outdoor time, and handwashing before lunch. The inspector discussed the following with the provider: children?s records, documenting identity, attendance for each separate group of children, and medication authorizations.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
2EVIDENCE:
1. Two medications, Allergy Relief (diphenhydramine) and AUVI-Q Auto Inject (epinephrine) were at the center for Child 3, and the authorization to administer these expired 3/2/2022.
2. Two medications, Allergy Relief (diphenhydramine) and AUVI-Q Auto Inject (epinephrine) were at the center for Child 4, and the authorization to administer these expired 9/22/2021.
EVIDENCE:
1. The attendance record for the infant group did not record the arrival of all children. Seven children were in care and the attendance record showed six children in care.
2. The attendance record for the early childhood group showed that there were no children in care. Of the ten children observed in the group, the written record showed that they were not in the center. Staff 5 stated she marked them all off by accident.
