An unannounced focused monitoring inspection was conducted on 5/15/2024 in response to a self-reported incident.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation, the center failed to be maintained in a safe manner.
EVIDENCE:
The wooden bridge in the playground has two slats that have caved in, causing a trip hazard.

Plan of Correction: The slats are scheduled to be replaced this weekend, and in the meantime, teachers stand at the broken spot to keep children away from it.

Based on observation, the center failed to have sufficient resilient surfacing and fall zones on playground equipment.
EVIDENCE:
The resilient surfacing (playground wood chips) measured 4 ? inches deep under the monkey bars, which were 7 feet high. Use zones around the monkey bars were less than six feet ? in some areas the use zone of the zip line was directly under the monkey bars. Use zones were not at least 6 feet around the outside perimeter of the play structure.

The playground surfacing will be measured daily and fluffed up with a heavy fork. Additional surfacing will be purchased as needed. The zip line and 360-degree swing ?wrecking ball? will be removed before children can use the equipment.
The equipment will be moved and/or boundaries expanded for a use zone of at least 6 feet on all sides of the equipment. An allowable variance will be requested because some use zones will be a bit shy of 6 feet.
