Inspector Notes
A subsidy health and safety (SHSI) inspection was conducted on May 20, 2024. There were 29 children present with four staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-790-600-D-1
Based on review of four staff records, the center did not ensure all staff who work directly with children obtained current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care within 90 days of the date of employment or 90 days from vender approval.
Evidence:
The record of staff #3, employed 01/04/2024, did not have documentation of current CPR exceeding 90 days from employment.
Plan of Correction: Per the administrator: Staff may have completed CPR however it was not in their record. I will confirm they have current CPR and ensure documentation is placed in their record. if they do not have current CPR, we will schedule a class for them.

Based on review of seven staff records, the center did not ensure all staff who work directly with children shall obtain current certification in first aid appropriate to the ages of children in care within 90 days of the date of employment or 90 days from vender approval.
Evidence:
The record of staff #3, employed 01/04/2024, did not have documentation of current first aid certification exceeding 90 days from employment.
Plan of Correction: Per the administrator: Staff may have completed first aid however it was not in their record. I will confirm they have current first aid and ensure documentation is placed in their record. if they do not have current first aid, we will schedule a class for them.
Standard 8VAC20-790-600-F
Based on review of three staff records, the center did not ensure three staff completed, in addition to preservice, and orientation training, annually attend at least 16 hours of training to include the department?s health and safety update course.
Evidence:
The most recent completion of the department?s annual health and safety update course for staff #3 (employed 09/01/2020) was dated 04/26/2021. There was no current 2023 documentation of the health and safety updated training.
Plan of Correction: Per the administrator: I will have staff complete the required training and maintain it in their record.
