Inspector Notes

A self-reported monitoring inspection was initiated on 04/14/2022 and concluded on 07/29/2022 in response to a self-reported incident regarding injury reporting that was received by the department on 04/05/22. The inspector conducted an unannounced inspection on-site on 04/14/2022. Interviews with staff were completed and observations were made at the center. Additional interviews were conducted off-site and additional documentation submitted virtually. The evidence gathered during the investigation did not support non-compliance with licensing standards. There are no violations resulting from this inspection.
