Inspector Notes

A complaint was received by the department regarding allegations in the areas of supervision, staff/children ratios, staff qualifications, and daily activities. A complaint inspection was initiated on March 23, 2022 beginning at 10:00 a.m. A walkthrough of the building was conducted. There were 44 children in care under the care and supervision of 7 staff.

Staff interviews were conducted. Documentation was reviewed including six staff records.

The information gathered during the investigation did not support the allegations and the complaint was determined not valid. One violation unrelated to the complaint was documented.

Based on review of documentation, the facility failed to obtain documentation of Tuberculosis (TB) screening for staff prior to coming into contact with children. Evidence: Staff #1, start date 3/1/2022, did not have documentation of a TB screening in the file.
Plan of Correction: TB documentation will be filed as soon as possible.
