Based on interviews with staff, it was determined that the vendor failed to inform the department's representative no more than two business days of any injury to a child while under the vendor's supervision when a referral is made for outside medical treatment from a medical professional. Evidence: On 2/10/2023, Child A sustained an injury to their nose that caused bleeding and swelling. Child A's parent took the child to the pediatrician for follow up and reported this to at least two staff of the center the day of the injury.
