Inspector Notes
An inspection with the local agency was conducted in response to a self-report received by the licensing office regarding rough handling concerns. An inspection was initiated on July 21, 2023 beginning at 10:10 a.m. and ended at 11:30 a.m. with the director.
Interviews were conducted. Documentation including a staff record and one child's record were reviewed.
An exit interview was conducted by phone on 8/21/2023 with the assistant director.
Information gathered during the inspection determined non-compliance with applicable standards or law and a violation was documented on the violation notice issued to the program.

Based on interview and observation, the center failed to report any suspected incident of child abuse as required. Evidence: The center self-reported a rough handling incident to the licensing office but failed to notify the local department of social services as required. Documentation reviewed revealed the center labeled the incident as alleged child abuse.

Based on review of documentation and interviews, it was determined that a staff engaged in a forbidden action of rough handling when caring for children. Evidence: Staff #1 was observed by Staff #2 roughly handing Child #1, 4 years old. Staff #1 grabbed Child #1 by the right arm and sat Child #1 in their seat. Child #1 said ?ouch? and grabbed their arm. Staff #2 observed red marks on Child #1?s arm. Child #1 reported that Staff #1 ?had a grumpy face? when Staff #1 grabbed their arm.
