The Center is required to ensure that the written record of children?s injuries contained the staff present, the date and time when parents were notified; any future action to prevent recurrence of the injury; and staff and parent signatures or two staff signatures.
After reviewing 3 injury reports for Child #1, it was determined that one of the injury reports written for Child #1 was missing documentation of the staff present and a second signature. 2 reports were missing the date and time the parents were notified and any future action to prevent recurrence of the injury.

Plan of Correction: All center staff have been retrained on filling out an incident/accident report. Staff have also been given a sample to make sure they understand all areas that must be filled out
