During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site focused inspection was initiated and completed on 3-18-26, as a part of the provisional licensure period. The on-site inspection began at 1:10pm and ended at 2:40pm The inspector reviewed compliance in the areas listed above. There were 34 children present and 6 staff. The inspector reviewed 1 child?s records and 4 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4-1-26. A POC submitted after this date will not appear on the public website.

The Center is required to post the license and the findings of the most recent licensing inspection of the facility in a conspicuous place on the licensed premises.

1)The Provisional license was not posted.
2)The inspection completed on 2/2/26, which was the most recent licensing inspection of the facility, was not posted.
Plan of Correction: Director posted provisional license and correct inspection summary report.
Standard 8VAC20-780-40-M
(Repeat Violation)The Center is required to maintain a current and dated written list of children?s allergies, sensitivities, and dietary restrictions that is accessible to all staff in each group or area where children are present.

The list did not contain Child #1`s food restrictions.
Plan of Correction: New updated allergy list was created on Updated classrooms as well as in binders.
Standard 8VAC20-780-50-A
The Center is required to treat staff and child records confidentially.

There were records posted at the center dated 11/24/25 that contained confidential information for staff and children.
Plan of Correction: Management removed the summary report and posted the inspection summary for the inspection completed on 2/2/26
Standard 8VAC20-780-80-A
The Center is required to keep a written record of daily attendance for each group of children documenting the arrival and departure of each child in care as it occurs.


During inspection in the Infant classroom, there were 3 children present, but the written record reflected there were 2 children present.
Plan of Correction: Management went to classroom and spoke with teacher reminding her of the importance of making sure all forms of classroom check ins were noted correctly.
