Inspector Notes

Licensing Inspector conducted an unannounced Religiously Exempt Code (Annual/Monitor) Inspection on September 20, 2023. There were 19 children ages 2 1/2 to 4 years old present under the direct care of 5 staff. The Licensing Inspector reviewed 4 children?s records and 4 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the Center failed to have a staff person trained in the daily health screening and exclusion of sick children.

There was not at least one staff member present who has received training in the Daily Health Observations for children.

Based on discussion with the Center Director, the Center failed to install carbon monoxide detectors in each building built before 2015 and houses a child day program serving preschool-age children.

EVIDENCE:
