Inspector Notes

This monitoring inspection was initiated by licensing staff and concluded on 08/17/2022, between the hours of 9:30 am to 11:30 am. There were 12 children present (28 points), ranging from infant to preschool-age, with 3 caregivers supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of (12) children records, (6) household records, and (3) staff records were reviewed.

Information gathered during the inspection determined that there were (2) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the home.

Standard 8VAC20-800-100-A
Based on review, 1 of 12 children files indicated that the Family Day Home (FDH) did not obtain documentation of a physical examination (by or under the direction of a physician) within 30 days after the first day of attendance.

Evidence -
1. On the date of inspection, 08/17/2022, documentation of a physical examination for Child #12 was unavailable for review.
2. The start date for Child #12 was listed as 05/09/22. Child #12 was in attendance on the date of inspection.
Plan of Correction: Child #12 has an appointment next month. A copy of a physical examination will be obtained after this visit.

Based on review, 3 of 12 children files indicated that not all of the required information / documentation was contained within each record.

Evidence -
1. On the date of inspection, 08/17/2022, Proof of Identification was not available for Child #2, Child #3, and Child #12.
2. Their start dates were listed as: For Child #2 - 02/28/22, for Child #3 - 05/09/22, and for Child #12 - 05/09/22. All three children were in attendance on the date of inspection.
