Inspector Notes

This Subsidy Health and Safety Inspection (SHSI) was initiated by licensing staff and concluded on 03/30/2023, between the hours of 9:30 am to 12 pm. There were 12 children with 2 resident children present (33 points), with 03 caregivers supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined (2) non-compliances with the applicable standards, code, and/or law. Violations were documented on the Violation Notice issued to the center or home.

Standard 8VAC20-790-250-B
Based on review, 1 of 3 staff files indicated that within the first 90 days of employment or service all caregivers the Virginia Preservice Training for Child Care Staff sponsored by the Department of Education was not completed.

Evidence -
1. On the date of inspection (03/30/2023), a certificate of completion of the Virginia Preservice (10 hour) training was not available for review for Staff #2.
2. The start date for Staff #2 was listed as 08/16/2022.
Plan of Correction: The Provider will contact the Subsidy Vendor Manager to withdraw from the subsidy program.
Standard 8VAC20-790-250-F
Based on review, 1 of 3 staff files reviewed indicated that the annual department's health and safety update course was not completed for 2022.

Evidence -
1. On the date of inspection (03/30/2023), it was determined that a certificate of completion for the 2022 Annual Health and Safety Update course was not available to review for Staff #1.
2. This course is the 3 hours Health and Safety course.
Plan of Correction: The Provider will contact the Subsidy Vendor Manager to withdraw from the subsidy program.
