During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was conducted on 7/28/2025 from 1:41pm to 3:45pm. There were 49 children and 8 staff present during the inspection. The inspector reviewed 5 chlid records, 3 staff records, and 2 medications. The inspection included document review, interviews, observations, and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which is 8/7/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and out, shall be maintained in a clean, safe and operable condition.

The black protective coating on the steps of the slide structure on the larger playground had peeled off, exposing an approximate foot long section of rusted metal.
Plan of Correction: Maintenance has been notified about the protective coating on the playset. They are scheduled to come out to repair the structure.
Standard 8VAC20-780-400-B
Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflict.

On the date of the inspection, Staff #1 was overheard making a threat to Child #1, 6 years of age, when they were not listening.
Plan of Correction: Staff 1 has been formally counseled and retrained on appropriate and positive redirection.

The following actions or threats are forbidden: verbal remarks which are demeaning to the child.

On the date of the inspection, the inspector overheard Staff #1 telling Child #1, 6 years of age, that they were going to tell their parent not to buy them anything for their birthday beacuse they were not listening.
Plan of Correction: Staff 1 has been formally counseled and retrained on appropriate and positive redirection.
