Inspector Notes

An in person Renewal study was conducted today. There are 8 children present with 3 caregivers.
Children were observed playing with a variety of age appropriate toys.
6 caregiver/household member records were reviewed. All were complete.
4 child records were reviewed. See Violation notice for non-compliances.
First aid supplies, Emergency supplies, Emergency evacuation drills, diapering and bathroom supplies and fridge/freezer temperatures were all in compliance.

Standard 8VAC20-800-60-A
Based on a review of records, the provider did not maintain an up-to-date record for all enrolled children.
Evidence:
1 of 4 child records reviewed was missing a record. There was no record available for review.
(Child 4).
Plan of Correction: The parent has the child's record to get documents notarized. I will ask them to bring all of the forms back tomorrow.
Standard 8VAC20-800-60-B
Based on a review of records, child records do not contain documentation of all required information.
Evidence:
2 of 4 records reviewed was missing documentation of having viewed proof of birth documents. (Children # 2 ,#3).
Plan of Correction: I will ask the parent to bringing the birth certificates.
Standard 8VAC20-800-90-A
Based on a review of records, one child record does not contain documentation of immunizations prior to the first day of attendance.
Evidence:
1 of 4 records reviewed was missing documentation of an immunization record. (Child 1)
Plan of Correction: I will ask the parents to provide the shot record.
