Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 9/4/24 and completed on the same date, as a part of licensure period. The on-site inspection began at 10:35 and ended at 12:10. The inspector reviewed compliance in the areas listed above. There were 17 preschool children as well as 12 school age children present and 5 staff. The inspector reviewed 5 children?s records and 1 staff record on site. This inspection included .

Based on the review of 5 children's records, the center failed to get some of the required documentation.
Evidence: 1 out of 5 children's records is missing the proof of ID. Child 3
Plan of Correction: The information was requested, the family just moved and have lost the key to the safe where the birth certificate has been placed. They will bring it as soon as they find the key.
