During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on 2/3/2025 and completed on the same date, as a part of licensure period. The on-site inspection began at 10:15 and ended at 11:45. The inspector reviewed compliance in the areas listed above. There were 26 children present and 6 staff. The inspector reviewed 5 children?s records and 3 staff records on site. This inspection included, document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-2
Staff must have the fingerprinting done before starting to work.

Staff 3 is missing the criminal records check. staff has been employed for more than 3 months.
Plan of Correction: We submitted the background checks and the staff went and did her fingerprints in September 2024. We did not receive any criminal records check back and emailed OBI several times. We never got an answer and on December 17, 2025 we received an email saying they had closed her file due to insufficient information.
The staff is in the process of getting new fingerprints done.

Staff must be trained in using any specialized medication at the center.

The center has an Auvi-Q device and center staff have the basic MAT training but not the added Auvi-Q training.
Plan of Correction: We will make sure staff takes the additional training.

The center must have an annual approval from the health department meeting the requirements for water supply and Sewage disposal systems.

The center does not have an environmental health inspection.
Plan of Correction: I will call the health department and schedule the inspection asap.

Center shall implement monthly emergency drills.

The center did not conduct an emergency drill in January 2025.
Plan of Correction: We will make sure we have monthly drills.
