Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on September 18, 2024, as a part of the licensure period. The on-site inspection began at 7:30am and ended at 8:30am. The inspector reviewed compliance in the areas listed above. CDC: There were 11 children present and 3 staff. The inspector reviewed 4 children's records and 3 staff records on-site. This inspection included document review, tour of the facility and observations

Staff shall request at least annually, parent confirmation that the required information in the child's record is up to date.

The record for child #1 did not have a parents signature for an annual review of the child's record. Child #1 has been in care for over two years.
Plan of Correction: We will have the parents review and sign for confirmation they reviewed it.
