Inspector Notes

The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on July 8, 2023 regarding allegations of administration, special care provisions and emergencies, and programs. The inspection was initiated on 8/10/2023 and completed on 8/31/2023. On 8/10/2023, the inspector was on site between 1:05 pm to 2:35 pm. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, there were violations cited during the inspection regarding staffing and supervision. See the violation notice for more details.

Repeat Violation
Based on observation and staff interview the center did not ensure staff to children ratios are in compliance during the designated rest period. Children 16 months through 24 months of age: one staff member per 10 children.
Evidence: Staff confirmed that in the Cat 2 room there was 1 staff and 16 children with the youngest child in the group being 22 months (date of birth 10/8/2021).
Plan of Correction: A letter was obtained from the parent indicating their approval for their child to be transitioning prior to his actual birthday. A cognitive assessment accompanied the letter.
