Inspector Notes
An unannounced renewal inspection was conducted on March 28, 2023 from 12:10pm to approximately 1:45pm. There were 28 children and 6 staff members present. The Inspector reviewed six children?s records and three staff records. The Inspector observed the interaction of staff and children throughout the inspection. Children just finished their lunches and were observed during naptime. First aid kits and emergency preparedness documents were observed. Transportation is not provided at the center. Medication was observed. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program. The violations were discussed with the owner at the exit interview.
Violations
2Evidence:
1. In the 3 year old classroom, there are torn and damaged venetian blinds which are in reach of children. This poses as an injury hazard to the children.
2. In the school age classroom, the children?s table is damaged with two areas (approximately 1-2 inches) that have exposed chipping wood.
Evidence:
1. Child #1 (date of enrollment 8/16/2022) did not have the father?s work phone number listed.
2. Staff #1 confirmed that Child #1 did not have the father?s work phone number listed to the record.
