Inspector Notes

An unannounced monitoring inspection was conducted on 3/22/23 from at 10:15am. There were 79 (2yrs to 5yrs) directly supervised by 16 staff. The physical plant, 3 staff records, 5 children?s records, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, playing outside, and eating snack. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. Staff-to-child ratios were in compliance. The center was clean and organized. Based on review, the facility did not ensure that all applicants for employment obtain a sworn statement of disclosure prior to employment and then every 5 years thereafter.
Evidence: reviewed 3 staff records and found that staff 2 did not have documentation of a subsequent sworn statement of disclosure completed within 5 years of the previous statement. The most recent sworn statement was dated 9/22/2015.
