Inspector Notes

An unannounced, on-site Code Compliance inspection was initiated on 9/23/24and completed on 9/23/24. The on-site inspection began at 9:50am and ended at 11:55am. The inspector reviewed compliance in the areas listed above. There were 112 children present and 21 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included, document review, tour of the facility, interviews, observations, and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/2/24. A POC submitted after this date will not appear on the public website.

The Provider is required, prior to providing supervision to children and annually thereafter, to be certified by a practicing physician or physician assistant to be free from any disability that would prevent them from caring for children under their supervision.

For staff hired after 7/1/24, the Provider is required to request the results of a criminal history record check, a sex offender registry search, and a central registry check from all states in which staff members have resided within the last five years prior to employment. For staff hired prior to 7/1/24, the Provider is required to obtain the results of a criminal history record check from all states in which staff members have resided within the last five years prior to employment.

Evidence:
1)The record for Staff #1 did not contain documentation that a criminal history record check, a central registry check or a sex offender registry clearance from all states where they resided within the last five years was requested prior to employment.
2)The record for Staff #2 did not contain documentation of a completed criminal history check from all states where they resided within the last five years prior to employment.
