Inspector Notes

A monitoring inspection was conducted on 12/05/2023 between the hours of approximately 9:30 am and 10:30 am. There were 13 children present, ranging in ages from 3 years to 5 years, with 2 staff supervising. Children and staff were observed while tracing numbers and letters and playing with toys.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on documentation review, the center did not provide to the licensing representative evidence of public liability insurance coverage .
Evidence:
1) On the date of inspection, the most recent proof insurance document available for review was dated 09/29/2022-2023.

Based on record review, the provider did not ensure to annually, review with the parent the emergency contact information required to ensure the information is correct, and obtain the parent's signed acknowledgment of the review.
Evidence:
1) The review of the emergency information in the record for Child #3 was dated 11/18/2022.
