An unannounced renewal inspection was conducted from 9:45 - 11:45am with the director. There were 29 children, ranging from three years to five years of age, present with six staff supervising. Children were observed playing in the gym, eating snack, washing hands, reading books and playing outside on the playground. Snack served today: animal crackers and water. Three staff records, three child records, the daily attendance record, emergency drill log, fire and health inspections, and the written list of food allergies, sensitivities and dietary restrictions were all reviewed. Two emergency medications for two children were reviewed and two staff have current certification in Medication Administration Training (MAT). The Annex 3 classroom was measured for additional use and can accommodate 18 children.

Based on observation, the facility failed to ensure that hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children. Evidence: There was a bottle of bleach water solution sitting on an open shelf above the sink in the toddler classroom. There was a bottle of bleach water solution sitting in an unlocked cabinet above the sink in the pre-K classroom.
Plan of Correction: Will retrain staff to ensure that hazardous substances remain locked at all times.

Based on observation, and an interview with staff, the facility failed to ensure that when the center provides a snack it follows the most recent age-appropriate nutritional requirements of the USDA. Evidence: snack served today was animal crackers and water and did not contain a second component as required.
Plan of Correction: Will ensure that snacks served consist of two different components from the USDA food guidelines.

Based on review of the written attendance record, the facility did not ensure all children were included on the written attendance record. Evidence: In the room 6 (3's) class, 12 children were listed on the attendance record and 13 children were observed in the class (gym.)
Plan of Correction: Will retrain staff to maintain accurate attendance records.
