Inspector Notes

An unannounced renewal inspection was conducted on 03/20/2023 from 10:00am to 11:30am. There were 23 children ages 3 years old to age 5 years old supervised by 4 staff. The physical plant, outdoor playground area, programming, attendance, menus, 3 staff records, 3 children?s records, 2 medications with corresponding authorization records, emergency drills, emergency supplies, and policies were reviewed. Children were observed participating in literacy activities, sensory play, art and a building activity. Handwashing procedures were also observed. There was an adequate number of staff present with current certification in Medication Administration Training (MAT), CPR and First Aid, and Daily Health Observation (DHO) training. Areas of non-compliance are identified in the Violation Notice.

Based on observation, the center did not ensure that play equipment used by children had met the requirement that openings above the ground which allow a 3.5 inch by 6.25 inch rectangle to fit through should also allow a 9 inch circle to fit through.

Evidence: On the play structure, under a platform above the ground there is a rectangular gap in the equipment that measures 4.75 inches by 43 inches. A 9-inch circle cannot fit through the opening which could lead to an entrapment of a body part or risk of strangulation.
Plan of Correction: On 3/21/2023 the Preschool teachers were informed to keep children off the playground until further notice. Our department's Risk Wellness and Safety Specialist, who is a Certified Playground Safety Inspector visited the playground on 3/22/2023 to review. His review confirmed the violation and need for the playground to remain closed until the issue is resolved. Signage has been placed to keep children out of the space until the correction is complete. The Operations Manager at Chinn has contacted the Facility Maintenance Team to create and implement a solution to bring the playground into compliance.
