Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 4/9/21 and concluded on 4/16/21. The provider was contacted by telephone to initiate the inspection. There was 1 child and 1 provider present.

The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children's records were reviewed in order to ensure documentation was complete. Please submit an up to date inspection of your fireplace by 5/31/21.

The information gathered during the inspection determined one violation with applicable standards or law.

Based on a review of the most recent documentation of the inspection of the wood burning fireplace and chimney, it was determined that it has not been inspected annually ( once every 12 months).
Evidence:
The wood burning fireplace and chimney was last inspected on 10/29/18.
Plan of Correction: I will either get a new inspection or I will make it inoperable through permanently sealing it off.
