An unannounced renewal inspection was conducted today with the provider who had one child, 15 months of age present during the inspection. The crib was inspected and has a manufacturer date of 10/16. The provider has obtained First Aid/CPR certification through American Safety Health Institute and is due to expire on 11/6/23. The family pet, Doxie's rabies vaccination expires on 2/19/23. Please send me a copy of the up to date wood stove/chimney inspection by 5/18/22.

Based on interview and observation, it was determined that there was no documentation to show that the wood burning fireplace and chimney had been inspected annually.
