Inspector Notes
An unannounced monitoring inspection was conducted today with the provider from 2:10 pm to 2:45pm. There were no children present during this inspection. Four children and one household member's files were reviewed during this inspection.
The monthly escape drill log was up to date, there were no medications on site and there have been no injury/accidents.
The area of non compliance is outlined on the violation notice.

Based on a review of four children's files and an interview with the provider, it was determined that all children did have the insurance information on file.
Evidence:
There was no insurance information on file for child A, B, C and D. Children A , B and C enrolled on 6/23/22, child D enrolled on 5/15/21.
