Inspector Notes

An unannounced , on-site monitoring inspection was initiated on 11/08/2024 and completed on 11/08/2024. The on-site inspection began at 11:38 am and ended at 12:37 pm. The inspector reviewed compliance in the areas listed above. There were 21 children present and 4 staff. The inspector reviewed 4 children's records on-site and 2 staff records electronically on 11/15/2024.

Standard 8VAC20-780-160-C
At least every two years from the date of the first initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis screening.

Staff 1 has not obtained an updated TB screening or test every two years as required. The TB screening on file expired approximately 5 months prior to the inspection date.
Plan of Correction: Staff 1 obtained a TB test on 11/18/2024.
Standard 8VAC20-780-260-A
Repeat Violation/Systemic Deficiency
The center shall provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction.

The most recent fire inspection, from the Norfolk Fire Marshall, was conducted on 07/03/2019.
Plan of Correction: Administrative staff stated that contact is being pursued with the community center to ascertain the status of the fire inspection for the facility.
