Inspector Notes

A complaint inspection was initiated on June 26, 2023, and concluded on August 30, 2023, in response to a complaint that was received by the department on June 5, 2023. The allegation is with regards to the direct care of the children at the center. Staff members were interviewed, and documentation was reviewed. This was investigated conjointly with another local agency. The evidence gathered during the investigation supports the allegations, therefore, the complaint is Valid. Violations regarding Programs (forbidden action) was cited. The violation was reviewed with the educational manager at the time of the completion.

Based on observation and record review, it was determined that the center did not ensure that the following actions or threats thereof are forbidden; roughly handling a child.
Evidence:
1. On May 31, 2023, the injury report written by staff documented that Staff #1 walked over to Child #1 and slammed Child #1 abruptly onto the carpet in the classroom.
2. In watching the video footage that is marked and dated May 26, 2023, at 10:39am, Staff #1 was observed to pick up Child #1 from the floor, swinging the child up off the ground, to then slam the child on the carpet. The audio from the video has Child #1 crying and screaming during this incident.
Plan of Correction: The center responded with the following: Staff #1 was suspended until further notice. With regards to the other staff members present, they were temporarily suspended and re-trained with regards to redirecting children, supervision, classroom management, and behavior management.
