Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on December 3, 2021 and concluded on December 3, 2021. The director was contacted by telephone and a virtual inspection was conducted. There were 52 children present, ranging in ages from three years to nine years, with eight staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and four staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of medications and authorizations, the center did not ensure when an authorization for medication expires, the parent was notified that the medication needs to be picked up within 14 days or the parent must renew the authorization.

Evidence:

The medication authorization for child #5 expired 09/28/2021, The medication had not been returned to the parent nor had the parent renewed the medication authorization form.
Plan of Correction: Per the administrator: We will have the parent complete a new authorization form or send the medication home with the parent.
