Inspector Notes
A monitoring inspection was conducted on June 24, 2024. The inspector was on site from approximately 9:00 am until 11:40 am. The director was present during the inspection. There were 28 children present, ranging in ages from three years to 12 years, with seven staff Supervising. Children were observed in a variety of activities including art (creating pirate ships, mermaid tails and crabs), building with magnetic blocks, circle time with music and bells, puzzles, and creative play with a grocery store center. Interactions between staff and children were positive.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations
3Evidence:
1. The record of staff #2, employed 11/08/2023, contained documentation of a negative tuberculosis screening dated 11/10/2023, exceeding the first day of employment.
2. The director acknowledged the TB results for staff #2 were documented after the first day of employment.
Evidence:
The record of staff #5, employed 12/28/2011, contained documentation of a tuberculosis
screening dated 06/01/2022, exceeding two years. The administrator stated a follow-up
tuberculosis had not been obtained.
Evidence:
1. The medication log documented that child #9 was administered a prescription medication from 06/05/2024 through 06/21/2024.
2. The medication authorization on file for child #9 contained only the first page with the physician?s authorization. The second page was missing and there was no authorization from the parent or guarding to administer.
3. The director acknowledged the medication authorization form was incomplete and did not contain the parent authorization.
