Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contact, document review, and an interview with the provider.
A monitoring inspection was initiated on 1/20/2022 and concluded on 1/26/2022 with an exit interview. The provider was contacted by telephone and a virtual inspection was conducted. There were six children present, ranging from nine months to five years of age, with the provider supervising. The inspector reviewed compliance in the areas of administration, household members, physical health of caregivers and household members, caregiver training, physical environment and equipment, care of children, preventing the spread of disease, medication administration, emergencies and nutrition. A total of two child and one caregiver records were reviewed and in compliance.
The information gathered during the inspection determined non-compliance with one applicable standard or law and the violation was documented on the violation notice issued to the program.
Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office by 4:00pm on Tuesday, 2/1/2022.
