Inspector Notes
An unannounced renewal inspection was conducted from 1:15 - 2:15 pm with the provider. There were seven children in care, ranging in age from 11 months to five years of age, for a total of 15 points. The children were observed having free play and watching an age-appropriate video. Two child records and three provider/household member records were reviewed. The daily attendance and emergency drill log were reviewed. The provider is not Medication Administration Training (MAT) certified and cannot administer medication to children in care.

Based on a review of two child records, the provider did not obtain documentation that each child has received a physical examination prior to the child's first attendance or within one month after attendance. Evidence: the record for child #1 (date of first attendance: 2/22/22) did not contain documentation of a physical exam.

Standard 8VAC20-800-580-C
Based on observation, and an interview with the provider, the provider failed to ensure that all children have daily opportunities for one or more rest or nap periods. Evidence: the provider stated that the children don't take naps, or have a rest period daily.

Based on observation, and an interview with the provider, it was determined that the diapering surface is not cleaned and sanitized after each use. Evidence: the provider stated, and demonstrated, that Lysol wipes are used after a child's diaper is changed.

Based on observation, and an interview with the provider, the provider failed to maintain documentation of emergency evacuation and shelter-in-place drills as required: Evidence: Documentation of the provider's emergency drills did not contain 1) the method used for notification of the drill, 2) any special conditions simulated, 3) the time it took to complete the drill, 4) problems encountered, if any, and 5) the weather conditions (for emergency evacuation drills only.)

Based on observation, and an interview with the provider, the provider failed to ensure that the menu was dated, given to parents, listed any substituted foods, and kept on file for one week, as required. Evidence: the posted menu was for the week of 2/14/22 - 2/18/22.
