Inspector Notes

An unannounced monitoring inspection was conducted from 11:10 am - 12:00 noon with the provider. There were five children, ranging from four months to five years of age, for a total of 12 points. Children were observed playing with age-appropriate toys. Four child records, two caregiver/household member records, the emergency drill log, menu and daily attendance were reviewed. The provider is not currently certified in Medication Administration Training (MAT) and cannot administer medication to children in care. Standard 8VAC20-800-100-A
Based on a review of four child records, the provider did not obtain documentation that each child has received a physical examination by or under the direction of a physician prior to the child's first attendance or within one month after attendance. Evidence: the record for child #2 (date of first attendance: 7/19/2021) and child #4 (date of first attendance: 8/22/2022) did not have documentation of a physical examination.

Standard 8VAC20-800-430-A
Based on observation, children under two years of age had access to stairs. Evidence: When the LI arrived a 17 month old child was observed playing on the stairs. The provider stated that the group of children had been upstairs playing in a nearby room.

Standard 8VAC20-800-690-E
Based on observation, not all diapers were disposed of in a leak-proof or plastic lined storage system that is either foot operated or used in such a way that neither the caregiver?s hand nor the soiled diaper touches the exterior surface of the storage system during disposal. Evidence: 1) the provider stated that the trash can in the bathroom was used for soiled diapers, however there was not a liner in the can, 2) The provider stated that she takes the diapers outside to dispose of them.
