Inspector Notes
The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing office on 05/04/2022. Interviews were conducted with staff/administration relating to allegations that field-trip requirements were not followed. The evidence gathered during the investigation supports allegations, so the complaint is determined to be valid. Please contact me if you need further assistance. Thank you.

Based on discussions with administration the facility failed to maintain compliance with this standard as required related to walking field-trips.
Evidence:
The preschool classroom walked to the convenience store, located at the edge of their property, to obtain a surprise snack item from the store owner on 4-8-22. The parents were not notified of this outing until after the outing had occurred.
Field-trip standards require that parents be notified prior to any field-trip.

Plan of Correction: A blanket field-trip permisson form is completed upon enrollment. Moving forward, parents will be notified of any future surprise or scheduled walking field-trip as required to ensure continued complaince.
