Inspector Notes

The licensing inspector conducted an unannounced monitoring inspection, at which play groups, activities, lunch time, and rest time were observed. Children were observed; and staff and children files and other documentation were reviewed. 82 children were found to be in care, with 16 staff. Please contact me if you need further assistance. Based on review of staff records the facility failed to maintain complaince with this standard as required.

Evidence:
Staff record #1 did not have an updated PPD/TB test. The previous one had expired as of 11/05/2021.
Plan of Correction: An appointment will be made immediately to obtain an updated TB/PPD test as required.
