Inspector Notes

An unannounced renewal inspection was conducted 06/09/2022. There were 8 children, ages 14 months-4 years, and 3 staff present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, emergencies and nutrition. A total of 5 children?s records, 5 staff records, and 1 applicant/agent records were reviewed during the inspection. The inspector discussed the following with staff: infant formula, attendance, playgrounds and topical ointments. The inspector arrived at 8:15 am and departed at 10:45 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard 22.1-289.035-A
Based on record review and discussion with staff, the center failed to ensure that every employee at a child day center shall undergo a background check in accordance with the Code of Virginia prior to employment and every five years thereafter.

Evidence: The record for Staff 4 contained a documented hire date of 12/11/2013. The most recent search of the central registry was dated 04/06/2017. The most recent criminal record was dated 04/14/2017. There were no repeat background check documents obtained in 2022.
Plan of Correction: The staff is scheduled for fingerprinting and a central registry will be sent.
Standard 8VAC20-780-160-C
Based on record review and discussion with staff, the center failed to ensure that at least every two years from the date of the initial screening or testing, staff member shall obtain and submit the results of a follow-up tuberculosis screening.

Evidence: The most recent tuberculosis test/screening for Staff 4 was dated 02/02/2017; there was not a follow-up test/screening for 2019 or 2021 available for review.
Plan of Correction: Will get an update TB.
Standard 8VAC20-780-40-K
Based on discussion with staff, the center failed to develop written procedures for prevention of shaken baby syndrome or abusive head trauma.

Evidence: Staff 3 stated that the center did not have written procedure for prevention of shaken baby syndrome or abusive head trauma.
Plan of Correction: Will write the procedures.
Standard 8VAC20-780-530-A
Based on observations, record review and discussion with staff, the center failed to ensure that at least one staff member in each classroom or area where children are present shall have a current certification in CPR and first aid.

Evidence: Staff 4 was the only staff member in the classroom located in Building C from 6:30 am to approximately 10:00 am; Staff 4 does not have a current CPR or first aid certification.
Plan of Correction: Will get the certification.
Standard 8VAC20-780-530-C
Based on observations, record review and discussion with staff, the center failed to ensure that there shall be at least two people who are currently certified in CPR and First Aid on the premises during the center?s hours of operation, on fieldtrips, and wherever children are in care.

Evidence:
1. Staff 4 was the only staff member on the premises from 6:30 am to 7:30 am; Staff 4 does not have a current CPR or first aid certification.
2. Staff 1 was the only staff on the premises with a current certification in CPR and First Aid from 7:30 am until approximately 9:00 am during the 06/09/2022 inspection.
Plan of Correction: Will get the certification.
