Today an unannounced Monitoring Inspection took place between the hours of 2:00pm and 3:40pm. There were a total of 54 children with 10 staff within the supervision guidelines. The children were observed taking a nap, handwriting and other organized activities. A complete inspection of the physical plant, children and staff records, evacuation drill log, and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Standard 8VAC20-780-240-A
Based on a review of staff records, all required staff did not complete The Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.

Evidence:
1. Staff #1 (doh: 6/29/2022) did not have documentation of completed the The Virginia Department of Education-sponsored orientation course on file at the time of the inspection.
Plan of Correction: I will have the staff member complete the required training.
Standard 8VAC20-780-340-F
Based on observation, children under 10 years of age were not always within actual sight and sound supervision of staff.

Evidence:
1. At approximately 2:28pm, Child A (4 years old) was observed walking up the hallway towards the front of the school and then opening the door to enter the Primary 2 classroom. There were no staff members observed to be supervising Child A until Child A entered the Primary 2 classroom.
Plan of Correction: We will make sure children are always within sight and sound supervision at all times.

Based on a review of records, the center did not complete a practice evacuation drill each month.

Evidence:
1. There was no documentation of a practice evacuation drill completed for the month of September 2022.
2. The Director stated that a practice evacuation drill was not completed in the month of September 2022.
Plan of Correction: We will complete a monthly evacuation drill moving forward.
