Inspector Notes

Today an unannounced Monitoring Inspection took place between the hours of 12:35pm-1:50pm.There was a total of 50 children with 7 staff within the supervision guidelines. The children were observed reading a book, having circle time, preparing for lunch/rest time and other organized activities. A complete inspection of the physical plant, children and staff records, evacuation drill log, and emergency materials were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice. Thank you for your cooperation during the inspection. Standard 8VAC20-780-270-A
Based on observation, areas and equipment of the center, inside and outside, were not maintained in a clean, safe and operable condition.

Evidence:
1. There were multiple wooden tables with shaky legs (unstable) in the Primary rooms 1 & 2.
2. The step trash can in the Toddler classroom was not in operable condition. When pushing down on the step, the lid of the trash can would not open.
Plan of Correction: We will replace the tables and purchase a new step trash can.
Standard 8VAC20-780-350-C
Based on observation, when children are in ongoing mixed age groups, the staff-to-children ratio and group size applicable to the youngest child in the group was not applied to the entire group.

Evidence:
1. At approximately 12:15pm, Teacher 2 went on a lunch break, leaving Teacher 1 alone in the classroom with 16 children (ages 3-6 years old). Teacher 1 was out of ratio for approximately 45 minutes, from 12:15pm-1:00pm.
Plan of Correction: This was corrected during the inspection when the teacher returned from their lunch break. Moving forward we will make sure all classes are in the required ratios at all times.
