Inspector Notes

An unannounced, on-site monitoring inspection was completed on 8/26/2024. The on-site inspection began at 12:30pm and ended at 1:45pm. The inspector reviewed compliance in the areas listed above. There were 44 children present and 11 staff. The inspector reviewed 4 children?s records and 4 staff records on-site on 8/26/2024. This inspection included document review, tour of the facility, interviews, and observations. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were issued. The violations can be found on the Violation Notice.

Based on observation, hazardous substances such as insecticides, and pesticides were not kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. Flying insect trap pesticide Plug-Ins were found in all 3 Primary classrooms, as well as the Toddler classroom. They were plugged into wall outlets that were low to the ground and within the reach of children. The packaging of the insect pesticide plug-ins stated "Keep out of reach of children."
Plan of Correction: All pesticide plug-in traps were immediately removed from all 4 classrooms during the inspection.
